ASSESSMENT & REFERRAL
We identify areas of spiritual, social and economic need
The Assessment & Referral team utilizes professionals and trained volunteers working as case managers to assist in identifying areas of spiritual, social and economic need through family assessments. The appropriate services are identified through on-site programming and community referrals. The Assessment & Referral team is assigned with the responsibility to perform individual and family assessments and developing individualized service plans for referrals and assistance with food, clothing, shelter, counseling, job training, and other related services - both internally and externally.
How to Make a Referral to Changing Lives
- Referrals to Changing Lives must be made through the Department Administrators and/or Elders ONLY.
- When the administrator is informed about a need, the administrator will EMAIL a completed referral form to the Changing Lives Department Director.
- Referral forms CANNOT be placed in the Changing Lives mail box at the church.
- The Changing Lives Department Director with contact the client directly to do a Needs Assessment.
- After the needs assessment is conducted, the client is then referred to the appropriate Changing Lives department for services.
- If the services are not provided within Changing Lives or VCMI, the client will be referred to an appropriate community organization that can provide the necessary services.
